Qualifications for Assistance:

SIS is committed to help people receiving breast cancer treatment that needs financial assistance and resides in the Greater Rochester, Buffalo, and Syracuse, New York area counties including:  Monroe, Genesee, Livingston, Onondaga, Oneida, Ontario, Orleans, Oswego, Seneca, Steuben, Yates, Wayne and Wyoming, Erie and Alleghany.

 

How We Can Help:

SIS provides financial support to be assist with daily living expenses such as: rent/mortgage; transportation; utility expenses; medical expenses such as co-pays; mastectomy products, wigs and headwear, & lymphedema products.

 

The Grant Process:

Each application is reviewed by the grant committee on a case by case basis.  A patient can only receive one grant annually.

Documentation Required:

All patients requesting assistance must fully complete an application which includes medical, financial and personal information. Incomplete applications will not be processed and will be returned to patient for completion. All information will be kept confidential and accessible only to authorized SIS staff, grant reviewers, and board members for the means of establishing eligibility.

Factors considered will include the severity of need and the resources currently available to SIS.

Future Eligibility:

Patients are eligible to re-apply to SIS for additional assistance 12 months or more after the date of their last grant application. We request that the patient submit a new application at that time including the medical provider verification form. Previous assistance does not guarantee future assistance.

 

Payment by SIS:

It takes approximately 60 days to approve the grants. If there are questions regarding your application or if additional information is needed, we will contact you at the time of review.
